Transition preparation focuses on building upon a student’s positive previous work while actively preparing them to transition out of Second Nature. Students receive more freedom and privileges as they demonstrate leadership, personal responsibility, appropriate interactions with others, problem solving skills, effective communication skills, awareness of strengths, and reconnection with family.
Aftercare plans should be in place 2 weeks prior to program completion as therapists assist students in accepting, preparing for, and transitioning successfully to the next step in their journey.

Parents are prepared for transition with the therapist. Discussions regarding expectations, reunion, transition challenges, and the family's unique therapy issues are facilitated during in-field transition experience. Due to the intense challenge of practicing new skills and roles for both parents and students, siblings are not invited.
